Appointments will be scheduled after insurance benefits have been checked.

If you have an HMO plan, we will have to have a pre-authorization approval number before we can schedule an appointment. 

Online consents and paperwork will be available in the Patient Portal or can be printed from the links below. 

Please have your paperwork completed prior to your appointment.
