Please contact our office to determine the appropriate intake forms needed for your appointment.

Appointments will be scheduled after insurance benefits have been checked.

If you have an HMO plan, we will have to have a pre-authorization approval number before we can schedule an appointment. 

Online consents and paperwork will be emailed after you have been scheduled.
